ICLS

Individual Community Living Supports (ICLS) is a bundled Elderly Waiver (EW) service, also available under the Alternative Care (AC) program, designed to help older adults (65+) remain safely and independently in their own homes. It provides a combination of supports—including reminders, cues, intermittent/moderate supervision, or physical assistance—tailored to each individual's needs.

Individual Community Living Supports (ICLS)

A Minnesota Elderly Waiver and Alternative Care service, ICLS offers bundled support—across six key areas—to help older adults live independently with reminders, moderate assistance, and social engagement. Eligible individuals can receive up to 12 hours of support per day, focusing on enhancing cognitive functioning, daily living tasks, household management, health and safety, and meaningful community involvement. Only certified, licensed providers deliver ICLS, based on a personalized plan developed in collaboration with the individual and care coordinator. Services are re-assessed annually to ensure they continue to meet evolving needs.
